COVID PROTOCOLS
- Obey all local laws and guidance for the program.
- If the activity is not permitted under local law, please follow the local guidance.
- If the activity is going to be done on a college campus, follow college campus rules.
- People should be properly masked (Double masks preferred).
- People should stay six feet away from others at all times.
VOLUNTEER AGREEMENT
I am aware of the potential spread of COVID-19 that could result in severe illness and potential death.
Therefore, I will, to the best of my ability, practice proper social distancing while engaging in volunteer work. I will follow local law, as well as any applicable campus/community guidelines.
I am certifying that I am not exhibiting symptoms of COVID-19, which include, but are not limited to:
Cough, Shortness of breath or difficulty breathing, Fever, Chills, Repeated shaking with chills, Muscle pain, Headache, Sore throat, New loss of taste or smell
Also, to the best of my knowledge, I have not: 1) been in close contact with others who have tested positive for COVID-19 during the past two weeks (friends, family, co-workers, etc.); 2) tested positive for COVID-19; 3) been currently living with someone or caring for someone who has been diagnosed with COVID-19; or 4) traveled to a foreign country with a level 2, 3, or 4 travel advisory by the US State Department or visited states or cities with known widespread transmission of COVID-19.
Should I become ill or start exhibiting any of these symptoms, I promise to self-report the volunteer leader and will leave the volunteer location. I will contact my primary care provider for direction.
Finally, if I am diagnosed with COVID-19 within 14 days of volunteering, I will notify Truth Initiative.
I acknowledge that even with precautions, any onsite activities may result in the spread of COVID-19 or other risks of injury.
